Claim Process

Our dearest Sequis Life’s loyal customers, we gladly inform you our fastest and most efficient claim process procedures:

  1. If you want to make a claim process, you can take the claim form at these certain locations: (a) Sequis Life’s head office at Jakarta, (b) Regional Service Center (RSC) at 3 major cities : Surabaya, Medan, and Bandung, (c) our nearest branch offices and (d) our website: www.sequislife.com
  2. After you have the claim form, please fill out your necessary data in the claim form. Please make sure that you had completed all the data in the form, in order to help us making the claim process runs smoothly. The person who take care the Insured is obligated to fill out the Description Letter from the Doctor.
  3. Afterwards, please kindly return the forms and all supporting documents to our National Service Center at Jakarta’s head office or Regional Service center (RSC) at Bandung/Medan/Surabaya or our nearest branch offices or you can send it directly to PT Asuransi Jiwa Sequis Life Head Office UP: Claim & Branch Operation Department (Sequis Center 5th Floor, Jl. Jend. Sudirman No. 71, Jakarta 12190, Indonesia).
  4. The Claim & Branch Operation Department will be in charge to process the entire claim requests. For Health Insurance Claim, it will take 7 days process procedure after the entire documents are received by our Head Office. Meanwhile, the Death Insurance Claim request will take 3 days process procedure after the entire documents are received by our Head Office. For claim that needs further investigation, Sequis Life will need more time to complete the process.
  5. When the claim request has been approved, the claim fund will be transferred to the Customer’s bank account.
Description:
Death Insurance claim consisted of 2 types which is reviewed based on policy contract period until the death risk stand point, which are:
  1. Contestable period or the policy which is still on trial period. In case where claim request occurs before 2 (two) years period, then the policy needs a further investigation.
  2. Non Contestable period or policy is no longer on trial period. For policy that is over 2 year’s period, after the entire documents completed, the claim request will be paid to the customer without any further investigation.
  • For claims that still need further investigations, we ask for your cooperation and understanding.
  • Before you sign the Insurance Request Letter (SPA), you should read carefully all the written contents in Insurance Request Letter (SPA), is it already completed and truthful according to all the facts. The completeness and truthfulness of the Insurance Request Letter will have a great role in our decision on the claim request as well impact our responsiveness of your claim process procedure.
